A1-GA Healthcare Agency ("A1-GA", "we", "us") is a Georgia home care provider. This website gives general information about Structured Family Care (SFC), Georgia's Elderly and Disabled Waiver Program (including CCSP and SOURCE), and our services. It is also a way to contact us.

Browsing this website, calling or emailing us, or submitting our eligibility form does not by itself make you a client of A1-GA or create a care arrangement. That begins only once enrollment is complete and the required agreements are in place.

Our eligibility form

Our eligibility form collects health information so we can review your situation. It is hosted by Jotform, a HIPAA-compliant form service, separate from this website, and we protect what you submit as protected health information under HIPAA. When you use the form, Jotform's own terms and privacy policy also apply. Please give accurate information so we can review your situation correctly.
