What this policy covers

This policy covers this website and our online eligibility form. It is not our HIPAA Notice of Privacy Practices. If you become a client, how we use and protect your health information is also described in our Notice of Privacy Practices, which we provide at enrollment and on request.

Information we collect

Health information, through our eligibility form

When you complete our eligibility form, we collect the information you provide. This can include your name and contact details, your county, and information about your loved one's health and Medicaid status. This is protected health information (PHI), and we collect, use and protect it in line with HIPAA.

The form is hosted by Jotform, a HIPAA-compliant form service, on its own website, separate from this one. What you enter goes directly to A1-GA through Jotform. It is never sent to, stored on or visible to this website, and we do not place analytics or advertising trackers on the form. Jotform's own privacy policy also applies when you use the form.

Website usage (Google Analytics)

We use Google Analytics to understand how people use the site, such as which pages are visited, how visitors arrived (for example, from a search engine), the type of device and browser, and approximate location (city or region). Google Analytics sets cookies in your browser to tell visits apart. It does not identify you by name, and Google Analytics 4 does not log or store IP addresses.

We have turned off Google Signals and advertising personalization, so this data is not linked to Google accounts or used for advertising. We do not send Google Analytics anything you type, and we do not use it on our eligibility form.

Hosting and security (Cloudflare)

The website is hosted on Cloudflare, which processes standard technical information, such as IP address, browser type and the pages requested, to deliver the site and protect it from abuse. We may also use Cloudflare Web Analytics, which counts visits without using cookies.

Information you choose to send us

If you call or email us, we receive the information you choose to share, such as your name, phone number, email address and the reason you are contacting us. We use it only to respond to you and to help with your inquiry.

Keeping health information off this website

• This website itself has no forms and does not ask for, receive or store health information. We collect health information only through our HIPAA-compliant eligibility form.
• We do not use advertising pixels (such as the Meta Pixel), heatmaps or session recording.
• We do not sell or rent personal information, or use health information for advertising or marketing.

Our website is also technically configured so that it cannot submit or embed forms, which keeps health information on the secure form and off this website.

Google Analytics uses cookies. You can block or delete cookies in your browser settings, or install Google's Google Analytics opt-out browser add-on. The website works normally without these cookies.

Links to other websites

This website links to other websites, such as Georgia.gov and the Georgia Department of Community Health. Their privacy practices are their own, and this policy does not cover them.

Security

We use reasonable safeguards to protect information, including an encrypted (HTTPS) connection for the entire website. No method of transmission or storage is completely secure, so please do not email us detailed health information. Use our eligibility form or call us instead.

Children

This website is intended for adults. We do not knowingly collect personal information from children under 13. If you believe a child has sent us personal information, contact us and we will delete it.
